Assistance Application Consent

and Privacy Notice

Effective Date: 10/13/2026

1. Purpose
Climbing Together Inc. (“we,” “us”) provides one-time financial assistance to eligible households in Orange and Seminole Counties. This notice explains what information we collect from you, how we use it, who sees it, and what you agree to by applying.

Information We Collect
We collect the information and documents you provide in the application. This includes your contact details, address, household size, income, the bill you need help with, your explanation of your need, any connection you have to Climbing Together, and the documents you upload.

How We Use Your Information
We use your information only to decide whether you qualify, to process an approved payment, to keep the records the law requires, and to report to our board without your name.

Who Sees Your Information
Only the people who review and process applications see your file. A reviewer who knows you does not vote. We do not sell your information. We may share it only with the vendor we pay, as needed to make the payment, or when the law requires it.

Verification
You authorize us to confirm the information and documents you submit. This may include contacting your landlord, utility company, lender, or provider to confirm the amount owed and the payment details.

Payments
If your application is approved, we pay the vendor directly. We do not pay the applicant. Approval is not guaranteed, and the amount we can give depends on the standards we follow and the funds available.

Documents and Deadlines
All required documents must be received within 7 days of your application. On day 8, an incomplete application is closed. You may apply again.

Accuracy
You confirm that the information and documents you give us are true and complete. We may deny or close an application that is false or incomplete.

Photos, Images, and Your Story
Sharing your story and images is optional, and it does not affect your application. If you allow it, Climbing Together may use your image and approved story to raise awareness and funds. You understand that sharing your experience may help inspire others in similar situations. We will show you the exact words of your story and use them only after you approve them. You may withdraw your permission at any time by writing to us. Anything already published may not be removable from places outside our control.

Retention
We keep your records for the length of time the law and our records policy require, and then we destroy them securely.

Your Questions
For questions or to correct your information, contact Climbing Together Inc., PO Box 181644, Casselberry, FL 32718.

Signature
By typing your name on the application, you confirm that you have read this notice and agree to it. You are electronically signing this notice, and your typed name has the same effect as your handwritten signature.
